Founded by Dr. Freddy Camuzzi, we have been in practice for more than 20 years. Our practice currently includes two university trained Urologists who combine advanced techniques, skill and experience in the compassionate treatment of your urological condition. Our mission is to provide our patients with superior healthcare in a compassionate and comfortable setting. Our staff is highly qualified and experienced in dealing with conditions of the kidney, bladder, prostate, male and female urinary tract.

NEW PATIENTS
General Information- Scheduled office hours are 9:00 a.m. to 5:00 p.m. Monday through Friday.
- Main Office Telephone (727) 345-2274.
- New patients referred by their Primary Care Provider are typically seen same day or within 24 hours.
- Appointments will receive reminders 24 hours prior to the scheduled appointment by telephone. If you would like to reschedule your appointment kindly give 24 hours notice.
- New patients can shorten wait times by completing intake forms prior to coming to the office. All intake and follow up forms are available for download and printing from the website (FORMS).
- All Cell phones and Pagers must be turned off while in the office as they may interfere with some of our electronic equipment and may disturb other patients.
- Pets are not permitted in the office.

All prescriptions and authorizations for renewals should be requested during normal office hours.

Forms
New patients can save considerable time by dowloading and completing their new patient information packet before coming to the office. In addition information about common procedures can be downloaded and viewed prior to office visits. All forms require Adobe Acrobat Reader to view.
